[Enhancement]: Update OpenSRP web with the latest OpenSRP branding

Use case/Problem Statement

• The OpenSRP logos have been updated recently, we need to update the default application to brand the latest logos. This involves updating the following areas
  • OpenSRP icon for the web app
  • OpenSRP logo on site favicon

Acceptance criteria

• The OpenSRP logos on the device should match the new logo
• The site favicon should match the new logo
